Mission
State certified case managers assess the living situations of frail elderly citizens and design care plans to enable them to remain independent and in their own homes.
Goal
To determine the needs of frail elderly clients, link them with appropiate in-home services and then monitor those services over time.
Description of program
NENAAA care managers assess all aspects of a client's life, including:
- Activities of daily living
- Finances
- Social interaction
- Transportation
- Cognitive abilities
- Nutrition
- Overall Health
- Home Safety
After completing the assessment interview, a care manager will make recommendations about the appropriate community resources to meet any needs. The client makes the final determination about services that may be enlisted. Care managers then monitor the living situation of their clients and will be available to assist in solving any future problems that may arise.

Over 400 per month with a total caseload of nearly 800 clients. - Fee
Fees for care management services are based on a sliding scale, set by the Federal Poverty guideline. An individual's income must be more than 150 percent of the poverty guideline before a care management fee is charged. No one is denied service because of an inability to pay. - Eligibility
Any person age 60 or older who resides in the 22 county area. Individuals outside NENAAA boundaries will be referred to the appropriate Area Agency on Aging. - Phone Number
