Personal Care Grant Program

Description of service

The Personal Care Program provides funding for individuals 60 and older who are having difficulties maintaining personal care tasks due to physical or mental disabilities whether long term or short term.  This grant is for personal care only – including bathing, toileting, dressing, etc.

Goal

To enable individuals 60 and over to remain independent by assisting with personal care tasks they may no longer be able to perform.

Eligibility

Recipients must be age 60 or older.  A NENAAA Care Manager, taking into consideration their functional needs, will determine eligibility.

Providers

What a personal care provider can do:

Assist or supervise personal care.  This includes help with bathing or showering, assistance with dressing, toileting, and transfers.

Providers are contracted by the client and paid by the client.  A Personal Care Grant, if the client is eligible, will reimburse the client up to the monthly contracted amount.  Names of Providers will be offered to the client.
