Aged and Disabled Medicaid Waiver Program

The Aged and Disabled Medicaid Waiver program is a service system based on the belief that people who require assistance in their home should have choices about the services they receive. After a thorough needs identification evaluation, eligible persons are offered the choice of receiving In-Home, Assisted Living, or Nursing Facility services.

Eligibility

Persons who are age 18 or older may be eligible for waiver services if they:

  • Are eligible for Medicaid
  • Agree to participate in an evaluation to determine their care needs and choose to accept support services
  • Have needs which are determined through the evaluation which would require assistance in the nursing facility setting
  • Can safely be served at home or assisted living at a cost not more than Medicaid would pay for nursing home care

Resources available through NENAAA Case Managers

  • Long-term assessment and design of a self-directed, person-centered plan of care
  • Individual counseling and support
  • Advice on available benefits
  • Coordination of all services coming into the home
  • Provide a health and social assessment
  • Help plan for needed services
  • Contact other agencies and care providers on behalf of client
  • Monitor how services are provided on a long-term basis

Covered Services

The Aged and Disabled Medicaid Waiver program allows Medicaid dollars to be used to pay for the following services:

Home Care/ChoreIncludes necessary personal care, companionship, housekeeping activities, meal preparation, essential shopping, and supervision.
Adult Day Health CareA structured program of activities in a supervised setting outside the client’s home which provides for health and social needs.
Respite CareTemporary relief for the caregiver from the stress of providing continuous care.
TransportationFor essential transportation needs.
Assisted LivingAn array of support services provided in a home environment.
Assistive Technology & SupportsSpecialized medical equipment, supplies and home modification, which will allow the client to increase their independence.
Home Delivered MealsProvides a meal prepared outside the client’s home and delivered to the client’s residence.
Personal Emergency Response SystemA communication device to access emergency help.

The Northeast Nebraska Area Agency on Aging accepts referrals from all sources. A visit is scheduled to meet with all eligible referrals. During the visit, the Services Coordinator and the client determine the needed services and support to keep the client at the highest level of independence possible.
